Navigating Mental Health During Motherhood: Medication, Misconceptions, and Compassionate Care

Anxiety, depression, and trauma during pregnancy can elevate maternal cortisol levels, which in turn may increase a baby’s baseline stress levels and risk of complications like preeclampsia and preterm labor. The current consensus: treating maternal mental health is essential for the well-being of both mother and child.
Jessie and Anne discuss the persistent myths surrounding antidepressant use in pregnancy and breastfeeding. Hofer emphasizes that SSRIs, like sertraline (Zoloft), have been thoroughly studied and are generally safe. In fact, there’s more evidence supporting their safety than for some commonly used antibiotics in pregnancy. Despite this, many providers, especially in rural areas or general practice settings, still perpetuate outdated beliefs. Hofer encourages women to seek out specialists in reproductive psychiatry if they feel their needs aren’t being met.
Medication is not a one-size-fits-all solution. Hofer explains the importance of finding the lowest effective dose—not the lowest possible dose—and reassures patients that being on medication doesn’t mean a lifetime commitment. Standard guidelines suggest maintaining treatment for at least a year before reassessing, but decisions are made collaboratively with each patient. Genetic testing is available for those interested, though it has its limitations and isn’t always more effective than trial and error.
Jessie and Anne also discuss the emotional barriers that keep women from seeking medication—fear of weakness, stigma, and preference for “natural” alternatives like supplements. Hofer respectfully challenges these beliefs, pointing out that many supplements lack the clinical research and regulation that psychiatric medications have. Comparing mental health treatment to diabetes management, she reinforces the idea that mental illness isn’t a personal failing—it’s a medical condition that deserves care.
